(Winners are determined by the percentage of body weight lost, not the number of pounds lost.)

2 Easy Ways to Enter!
- Come to Humphreys Diabetes Center (1226 River Street, Boise) on Jan. 7, 2012 from 8am – 1pm and join the Kick-Off Party (see more information in the event brochure.) You can bring along the Challenge 2012 Entry Form and Release .
- Or join during Continuing Registration between Jan. 8 and 22, 2012 at our Meridian office or at Ladd Family Pharmacy in Boise (Here’s the event brochure with addresses and times you can sign up!)
FINAL WEIGH-IN IS ON THURSDAY, JUNE 7.

Sign up as a team (5 or more people) for support and inspiration and get $5 off the registration fee! All team members must register and weigh-in together. Print and share copies of the Weight Loss Challenge Brochure and Challenge 2012 Entry Form and Release !

Important Information
Must be 18 years or older with no physical or medical restrictions. All registrants must present photo ID and wear shorts and T-shirts without shoes for initial weigh-in. Participants who have had weight loss surgery or procedures before or during the Challenge can participate but are ineligible for prize money. Sponsor employees and their spouses may participate but are ineligible for prize money. All proceeds from the Weight Loss Challenge benefit St. Luke’s Humphreys Diabetes Center.
